Submitting Applications for Return from Late Medical Leave of Absence

  • Applications for a return must be submitted to the Student Affairs Dean’s Office no later than six weeks before the semester in which the student wishes to return. Applications for returns will NOT be considered after the deadlines. Consult with your Community of Care Case Manager if you anticipate being on a leave longer than one semester.
    1. Applying for Reinstatement From Late Medical Leave of Absence
      • Complete the Return form below.
      • If you have questions, please contact the Student Affairs Dean’s Office to speak to a Case Manager at (310) 338-3756.
    2. Compose a Return to Academic Studies Letter
      • Student must write a letter to the Case Manager describing:
        • How the student has used the time away during the leave.
        • What the student intends to do to support health and well-being upon return to LMU campus.
    3. Revise Release of Information Form
      • Sign Release of Information form allowing Student Psychological Services or Student Health Services and your Case Manager to contact your health care providers to clarify any questions. Submit the revised Release of Information form to Case Manager.
    4. Complete Student Readiness to Return from Medical Leave of Absence Form
      • Student must ask the treatment provider(s) to complete the Student Readiness to Return form.
    5. Schedule a Return Meeting
      • The student is to contact their Case Manager to schedule an appointment for a return meeting with Student Psychological Services or Student Health Services to discuss recommendations upon the return to LMU campus and provide documentation of treatment efforts/resolution.
  • Return Assessment From Late Medical Leave of Absence
    • The SPS/SHS staff member and Case Manager will review all documentation provide completing the return assessment. During the meeting with SPS/SHS and your Case Manager, the student will be asked to discuss the treatment received while on leave. Additionally, the student will be asked to describe plans made for follow-up treatment upon return to LMU campus.
  • Determination
    • The Dean of Students will make a determination regarding a return based on the recommendation provided by the healthcare provider(s). A return is based on the student's readiness to manage an academic course load and safely function in our community. The Dean of Students will notify the student and other necessary University offices in writing of the determination, and if approved, whether any conditions will apply. If the return is denied, the student may appeal the determination.
      • If reinstatement is denied, the student may appeal the determination to the Student Affairs Dean’s Office (or designee) in writing within three (3) business days of the decision. The appeal should be made in writing and should set forth the basis for the appeal. The Senior Vice President for Student Affairs shall review the record and any additional information submitted by the student and will render a decision within ten (10) business days of receiving the appeal. The Senior Vice President for Student Affairs' decision shall be final.

Processing the Request to Return

  1. If you are returning within the four-semester RLA time period, fill out the form below.
  2. Meet with your Dean’s Office prior to registering for classes.
  3. Meet all financial aid deadlines for the academic year of the expected return and clear any financial holds.
  4. Check PROWL for any other holds on your account that need to be addressed, and contact the appropriate office for assistance in their removal.
  5. If you wish to reside on campus, contact the Student Housing Office and submit all the necessary forms to apply for housing and meet all deadlines.
  6. If you have disabilities, you may be eligible for reasonable accommodations and/or special services in accordance with the Rehabilitation Act of 1973, and the Americans with Disability Amendments Act (ADAA) of 2008. You are responsible for communicating requests for academic accommodations to the Office of Disability Support Services.
